學士班、研究生畢業離系手續 Procedure for Graduation Clearance of Department (for Undergraduate and Graduate Students)

一.系上系統(填寫系友資料及問卷)Department System (Complete Alumni Information and Questionnaire)

  服務單位填寫如下(不可空白):Fill in the following information (cannot be left blank):

  1.就業  填寫未來就業公司
    Employment: Enter the name of your future employer.
  2.繼續升學   填寫未來就讀學校(系所)
    Further Studies: Enter the name of the institution and program that you will be attending.
  3.服兵役  填寫:服兵役
     Military Service: Enter "服兵役" (Military Service).
  4.找工作  填寫:找工作
    Job Searching: Enter "找工作" (Job Searching).

  通訊資料部份，請填寫完整，俾利審核。Please provide complete contact information for verification.
  通過證照，若有通過的證照請填寫。If you have obtained any certifications, please specify them.
  離校學期：11301 Graduation Semester: 11301

二.學校系統  University System

 1.學生請登錄 學校首頁單一入口 的「教務資訊系統/畢業離校」頁面，
   確認本學期各科修課成 績皆到齊。  
  Students should log in to the "Academic Information System/Graduation Clearance and Leaving-School"   page via the university portal to confirm that all course grades for the semester have been recorded.
 2.研究生請另填寫「研究所離系手續單」經指導教授，交給所屬系所辦公室，再查詢離校狀態。
   Graduate students must also complete the "Graduate Department Departure Form," have it signed by their advisor, submit it to their department office, and then check their school-leaving status.
3.畢業生進入教務資訊系統查詢離校狀態，點選「畢業離校／離校狀態查詢」，更新畢業生離校通訊方式確認，並填寫調查問卷。
Graduates should check their graduation/departure status through the "Graduation and /Status Inquiry" section in the Academic Information System, update their contact information, and complete the required surveys:
*「意向調查卷」 (共九題): Career Intent Survey (9 questions).
*「UCAN職場共通職能診斷」 (共五十四題): UCAN Common Workplace Competency Assessment (54 questions).
若註記為"N"之單位，表示該單位的離校手續尚未完成，請電洽或親自該單位辦理。
If any department is marked "N," it indicates incomplete clearance procedures. Please contact the relevant department by phone or in person.

4.成績全部到齊及離校狀態全部顯示" Y "者，請持「學生證」，研究生還需要帶「論文考試結果通知書」正本，至註冊組領取畢業證書。
Once all grades are recorded, and the clearance status is marked "Y" for all departments, students may proceed to the Registration Office to collect their graduation certificates.
*Required Documents: Student ID card.
*Additional for Graduate Students: Their original copy of the "Thesis Examination Result Notification."

學生證不會收回，在學生證上註記後將歸還本人。
The student ID card will not be confiscated. It will be marked and returned to the student.
